Crisis Stabilization & Case Management
After a youth has been in contact with the Child and Youth Crisis Service, a REACH Crisis worker and their CYCS supervisor may determine that a short-term plan for support is put into place. This may be necessary when there are many complex issues that need to be resolved. By providing this short-term support, often a crisis can be stabilized until other resources can be found to help the child or youth. This may involve connecting families and youth together with other agencies or staff who can help them.

A Crisis Stabilization Case Management Worker will help:
  • Provide crisis stabilization/navigation services to children and youth with special emphasis on transitional age youth with complex needs (e.g., dual diagnosis).
  • Provide short-term service coordination to those same clients until other resources can be activated to support the child/youth and their families, preferably using the wraparound model when appropriate to do so.
  • Facilitate, participate and support clients and their families with other service providers including telepsychiatry services at Sick Children's Hospital and psychiatric services for children and youth being planned at MacMaster Children's Hospital.
  • To coordinate a system of alternative shelter for children/youth including emergency shelter (i.e., safe beds), respite services, etc.
  • Provide training and support to shelter and respite providers to better able them to support youth with complex needs.
  • To act as a resource to community partners with regard to working with children and youth with complex needs (i.e., dual diagnosis).
Am I eligible?
This service provides support and stabilization to children/youth who are under 18 years of age, and their families, who require longer support than can be provided by the Child and Youth Crisis Service. Referral to the Crisis Stabilization Case Management Service comes via the Child and Youth Crisis Service, which can facilitate a smooth transition to the Crisis Stabilization Case Management Worker that will be assigned to the client.

How do I access or find out more about this service?
The Child and Youth Crisis/In Home Intervention Supervisor is available to answer any questions regarding this service.

Initially a caller would be calling the Child and Youth Crisis Service for immediate assistance. A determination would be made after this service has been involved if a referral will be made to the Crisis Stabilization Case Management Service.
